Your ferritin comes back low again. You have taken the iron, you eat red meat every week, and six months later the number has barely moved. Nothing about your stomach feels wrong, so nobody has looked past the iron itself. A nutrient that stays low no matter how much goes in is one of the clearest reasons to look at absorption instead of intake.
Nutrients Reach Your Blood Only After Enzymes and Bile Break Food Apart
Chewing and stomach acid start things off, but the small intestine decides what you keep. Your pancreas sends in enzymes that cut protein, starch, and fat into pieces small enough to cross a cell membrane. Your gallbladder releases bile, which breaks fat into a fine suspension so the fat-splitting enzyme can reach it. Without both, a meal travels through mostly intact.
The intestinal wall is not smooth. It is folded into millions of finger-shaped projections called villi, each covered in far smaller bristles. Together they give the small intestine an absorbing surface many times larger than the tube itself.
The final step happens at the wall. Enzymes sitting on the surface of the villi make the last cuts, splitting milk sugar and table sugar into single units a cell can take in. Damage that surface and you lose the area available for absorption plus the enzymes that were sitting on it.
Each Nutrient Has Its Own Stretch of Intestine
Absorption is not spread evenly along the intestine. Iron and calcium are taken up in the duodenum, the first ten inches past the stomach. Most protein, sugar, and fat is handled in the long middle segment, the jejunum. Vitamin B12 and recycled bile acids are absorbed at the far end, in the terminal ileum.
That map is why the same amount of damage produces completely different deficiencies depending on where it sits.
Missing Enzymes, Damaged Villi, and Misplaced Bacteria Each Block Absorption
Malabsorption is a category, not a diagnosis. Something has gone wrong at one of three points on the path from meal to bloodstream, and which point it is determines what shows up in your labs and what helps.
Too Little Pancreatic Enzyme or Bile Leaves Fat Undigested
When the pancreas cannot make enough enzyme, fat is the first nutrient to fail, because fat digestion depends almost entirely on pancreatic lipase. Chronic pancreatitis, cystic fibrosis, pancreatic surgery, and pancreatic cancer are the usual causes.

Bile trouble produces the same greasy result from a different direction. Liver disease, a blocked bile duct, or surgical removal of the terminal ileum leaves too little bile acid in the intestine at mealtime. Fat then stays in droplets too large for enzymes to work on efficiently.
Celiac Disease and Crohn’s Disease Flatten the Absorbing Surface
In celiac disease, the immune reaction to gluten shortens and flattens the villi of the duodenum and upper jejunum. The absorbing surface shrinks, and the wall enzymes that sat on those villi are lost with it.
That second loss is why many people with untreated celiac disease cannot handle milk sugar. The lactase they need was sitting on the surface that got damaged. Removing gluten lets the villi rebuild, and milk often becomes tolerable again months later.
Crohn’s disease damages patchy segments and can strike anywhere along the tract, so the deficiency pattern depends on which segments are inflamed. Bacterial Overgrowth Consumes Nutrients Before You Absorb Them
The small intestine normally holds very few bacteria. When a large population settles there, the bacteria use vitamin B12 for their own metabolism before you get to it. They also strip the chemical groups off bile acids, which leaves the bile unable to hold fat in suspension.
Low B12, low vitamin D, and low iron can follow, though in milder cases the shortfalls stay small (Pimentel et al., American Journal of Gastroenterology, 2020, PMID 32023228). Iron is absorbed in the duodenum, and the duodenum is the first place celiac disease does damage. Iron deficiency that keeps coming back, or that barely responds to oral iron, is a reason to test for a cause rather than raise the dose.

B12 Needs a Stomach Protein and an Intact Ileum
Vitamin B12 absorption takes two working parts. Your stomach makes a carrier protein called intrinsic factor that latches onto B12, and the terminal ileum has the receptors that pull the pair inside. Stomach surgery, years of acid-suppressing medication, Crohn’s disease in the ileum, and bacterial overgrowth each break one of those steps.
The liver holds a large B12 reserve, so the deficiency builds quietly over years. It often shows up as numbness, tingling, or memory trouble before it changes a blood count.

Greasy Stools and Weight Loss Without Trying Point to Fat Malabsorption
Fat that is not absorbed leaves in the stool, and it changes what you see. The stool turns pale and bulky, smells strongly, floats, and leaves an oily film that is hard to flush. Many people notice this for months before mentioning it.
Fat carries more than twice the calories of protein or starch per gram. Losing it in the stool is where unplanned weight loss comes from, and it is why someone can eat plenty and still lose ground.
Vitamins A, D, E, and K Leave With the Unabsorbed Fat
These four vitamins dissolve in fat rather than water, so they cross the intestinal wall inside the same droplets that carry dietary fat. When the fat is not absorbed, they are not absorbed either.
Low vitamin A shows up as trouble seeing in dim light. Low vitamin D weakens bone over time. Low vitamin K shows up as easy bruising and cuts that keep bleeding. Unabsorbed Fat Binds Calcium and Raises Kidney Stone Risk
Normally the calcium in your food grabs oxalate inside the intestine, and the pair leaves in the stool. When unabsorbed fatty acids reach the colon, they bind that calcium first. Oxalate is left free, gets absorbed, and arrives at the kidney where it forms calcium oxalate crystals.
Unabsorbed bile acids make the colon wall leakier to oxalate at the same time, which compounds the problem (Witting et al., Nephrology Dialysis Transplantation, 2015, PMID 25701816). Eating calcium-containing food with meals rather than between them keeps calcium in the intestine at the moment oxalate is there, so more of the oxalate leaves in the stool.
Cutting back on spinach, beets, and almonds matters here in a way it does not for most people.
Stool and Blood Tests Separate a Pancreas Problem From an Intestinal One
Fecal Fat Confirms the Loss, Fecal Elastase Names the Pancreas
These two stool tests answer different questions. A fecal fat stain shows whether fat is being lost. An elastase test measures an enzyme the pancreas makes, so it tells you whether the pancreas is the reason.
A normal elastase result is good at ruling enzyme insufficiency out, while a low result in someone at low risk is often a false alarm that needs confirming before enzymes are started (Vanga et al., Clinical Gastroenterology and Hepatology, 2018, PMID 29374614). Keep Eating Gluten Until Celiac Testing Is Finished
Celiac blood testing measures your immune response to gluten. Take gluten out first and the antibody level falls, which can turn a true positive into a negative result and cost you the diagnosis.
The American College of Gastroenterology recommends that both the blood test and the biopsy happen while you are still eating gluten, so keep it in until testing is done (Rubio-Tapia et al., American Journal of Gastroenterology, 2023, PMID 36602836). Breath Testing and a Nutrient Panel Fill In the Rest
Breath tests catch the sugar-specific and bacterial causes. A lactose breath test and a lactulose breath test measure the hydrogen your gut bacteria make from sugar you did not absorb.

Smaller Meals and Medium-Chain Fat Get More Calories In While the Cause Is Treated
Fixing absorption means treating the cause. Gluten removal rebuilds villi over months. Prescription enzymes restore fat digestion. Antibiotics reduce an overgrown bacterial population. What you eat in the meantime decides how much of your food you keep.
Smaller Meals Match the Enzyme Supply You Have
A large meal delivers more fat and protein at once than a limited enzyme and bile supply can process, so a bigger share passes through undigested. Five or six smaller meals spread that demand across the day. People with pancreatic enzyme insufficiency usually report less gas, less urgency, and steadier weight on that pattern.
Medium-Chain Fat Crosses the Wall Without Bile
Fat comes in different chain lengths, and length changes how it is absorbed. Medium-chain fats, found in coconut oil and MCT oil, are water-soluble enough that they need no bile for emulsifying and very little lipase.
They pass through the wall and go straight into the portal vein toward the liver, skipping the packaging step long-chain fat requires. That is why they add calories when regular fat is being lost in the stool.
They are an addition, not a swap. Medium-chain fats carry no vitamin A, D, E, or K, and they supply none of the essential fatty acids your body cannot make. Large amounts at once cause cramping and loose stools, so small servings worked upward are easier to tolerate.
Enzyme Replacement Works Only When It Is Taken With Food
Prescription pancreatic enzymes have to be in the intestine at the same moment the meal is, because they can only act on food they are mixed with. Taken half an hour before eating, or after the plate is clear, they largely miss their target.
Spreading them across the meal, starting with the first bites, is the difference between treated and untreated fat loss (Whitcomb et al., Gastroenterology, 2023, PMID 37737818).
